Notifying a Claim

At Combined Insurance, we recognise that every claim made is the result of an unfortunate event, some very traumatic and life changing.

In Combined Insurance claims, we are always there to support and assist our customers, responding to their needs and not just the claim event. Our customers can expect the superior service and attention to detail that comes with every Combined Insurance policy. Our experienced team of claims professionals responds to each claim with a sense of urgency and care, providing technical expertise, responsive service, clear communication and consistent claims handling.

PLEASE ENSURE THAT YOU CURRENTLY HOLD POLICIES WHICH ARE FULLY UP-TO-DATE WITH PREMIUMS AND WHICH PROVIDE COVERAGE FOR THE EVENT YOU ARE LOOKING TO CLAIM FOR. ANY FEES PAYABLE, BY YOURSELVES, FOR COMPLETION OF THESE CLAIM FORMS WHICH ARE LATER DECLINED DUE TO NO POLICIES BEING ACTIVE WILL NOT BE REIMBURSED.

Please make sure that you…

 

  1. CLICK HERE for an Accident Claim Form, or CLICK HERE for a Sickness Claim Form

  2. print the claim form

  3. complete all the relevant sections and sign the claim form.

  4. carefully read, then sign and date, sections 6.2 and 6.3 (Access to Medical Reports and Statement of truth). Please check that your dates are accurate, as we assess your claim against this information. In section 6.4 (claims payment), don’t forget to provide us with the bank details of the account you'd like any claim payments paid into.

  5. ask your doctor to fully complete and sign section B.

  6. scan/photograph all completed pages of the claim form (and any additional sheets, including your hospital admission/discharge summary sheet(s) if you have been admitted as an inpatient to a ward).

  7. attach the scan/photograph of the completed claim form (and any additional documents) to an email and send to claims@uk.combined.com

If you are unable to print a claim form, or have any other questions, please call 0800 169 7733 (Monday – Friday, except bank holidays, 9am-5:30pm) free from a UK landline or mobile. Calls may be recorded for quality and training purposes.

  • Customer name
  • Address / email / phone number Policy number
  • Claim details
