Call us at 1300 300 480 Monday to Friday between 8:30am – 5:00pm AEST and AEDT. We can provide assistance over the phone and post out anything you may require to help action your claim.

Please note, all claim payments are now made electronically. To ensure prompt payment of claims, please include EFT details in the relevant section on the claim form.
Once downloaded, the relevant form should be completed in full by you and your doctor, signed by both, and posted to:
