UPDATE FOR POLICYHOLDERS IN GEORGIA

The Georgia Department of Insurance issued Bulletin 19-EX-05 and Bulletin 19-EX-06 declaring a state of emergency due to Hurricane Dorian. As a result of this weather event, and the potential displacement of Georgia residents, Combined Insurance customers residing in the disaster areas may be eligible for a due date extension of their Combined policy renewal premium payments due, as well as any deadlines to submit claim proof of loss. Please contact Combined's Customer Care Center at 800-225-4500 with any questions.

List of counties:
Appling, Bacon, Brantley, Bryan, Bulloch, Camden, Charlton, Chatham, Clinch, Echols, Effingham, Evans, Glynn, Liberty, Long, McIntosh, Pierce, Screven, Tattnall, Ware, and Wayne Counties.

UPDATE FOR POLICYHOLDERS IN NORTH CAROLINA

The North Carolina Department of Insurance issued Bulletin 19-B-04 declaring 14 counties as disaster areas due to Hurricane Dorian.  As a result of this weather event, and the potential displacement of North Carolina residents, Combined Insurance customers residing in the disaster areas may be eligible for a due date extension of their Combined policy renewal premium payments due, as well as any deadlines to submit claim proof of loss. UPDATE FOR POLICYHOLDERS IN TEXAS

On August 23, 2017, Texas Governor Greg Abbott issued an emergency bulletin (Bulletin B-0009-17) declaring multiple counties as disaster areas due to the effects of Hurricane Harvey.  As a result of the historic flooding caused by Hurricane Harvey, and the extensive displacement of Texas citizens, Combined Insurance customers residing in the Texas counties noted below may be eligible for a due date extension of their Combined policy renewal premium payments due, as well as any deadlines to submit claim proof of loss.  Please contact Combined’s Customer Care Center at 800-225-4500 with any questions.

The counties impacted are Angelina, Aransas, Atascosa, Austin, Bastrop, Bee, Bexar, Brazoria, Brazos, Burleson, Caldwell, Calhoun, Cameron, Chambers, Colorado, Comal, DeWitt, Fayette, Fort Bend, Galveston, Goliad, Gonzales, Grimes, Guadalupe, Hardin, Harris, Jackson, Jasper, Jefferson, Jim Wells, Karnes, Kerr, Kleberg, Lavaca, Lee, Leon, Liberty, Live Oak, Madison, Matagorda, Milam, Montgomery, Newton, Nueces, Orange, Polk, Refugio, Sabine, San Augustine, San Jacinto, San Patricio, Trinity, Tyler, Victoria, Walker, Waller, Washington, Wharton, Willacy and Wilson.

Filing a claim can be overwhelming. We want to make it and smooth as possible for you. You'll need this information handy:

  • Policy number
  • Policyholder name and birthdate
  • Copies of all claim-related documents like an Attending Physician statement or invoices


Other Ways to File a Claim

You can also submit your claim by mail or over the phone. Simply download the appropriate form from the Forms and Documents section on the left, print it out and complete it. Once all the information is filled in, just send it to the address or fax number below, or call us at the numbers provided.
